Guide to Person-Directed Planning
If you feel that you would benefit from help in planning for your life now and in the future, a person-directed plan may be of great value. The purpose of developing such a plan is to identify your life vision and goals and to find and use services and supports to meet your goals with the help of others in your life.
“A Guide on Person-Directed Planning – Creating a Good Life in Community” is a free resource that is available to explain how to create a plan.
Additional person-directed planning resources are also available at the Ontario Independent Facilitation Network website. This provincial network exists to inform, support and promote independent facilitation and planning in Ontario.
You may use up to $2,500 of your Passport funding to pay for the service needed to develop a person-directed plan.
